Omicron spreads – and with it the risk of contracting corona. Tests can detect infection. But how well do they work on the new variant? The most important questions and answers at a glance.

When should I test for Corona?

If you are sick feel, you should do a test. Feel Symptoms such as fever, cough, loss of smell or taste that can occur with a Corona infection, then they should isolate themselves and – if available – do a rapid test at home. The Federal Ministry of Health also advises you to call your doctor’s office for a PCR test To arrange. Because these tests are more sensitive than self-tests and can detect even small amounts of viruses.

Even if you have no symptoms and are fully vaccinated, Corona tests are now available at many locations Regulation: everywhere where rules according to 2G+ apply – such as in cultural and recreational settings, hairdressers, medical facilities and nursing homes. For unvaccinated people, testing is mandatory even at work and before using public transport. For Triple vaccinated the test obligation is void against it in most cases.

But even in situations where you don’t need to be tested, testing can be useful to prevent infecting others with Corona virus unnoticed. The Federal Ministry of Health advises [togethergegencorona.en] especially before Meeting with people from the risk group – so for example elderly or pre-diseased people, who are more likely to fall seriously ill with Covid19.

Free citizen tests again possible – So is tested in Berlin and Brandenburg now again free of charge

At least one Corona quick test is free again since Saturday – for everyone. The Senate refers to the twelve own test centers and to private agencies. In Brandenburg, many of the private test stations no longer exist.

Which quick tests for home work best?

Quick tests for home use are so-called antigen tests. They do not detect the genetic material of the corona virus, but a specific protein that is present in the virus. The advantage: it’s fast – the result is available within 15 minutes.

The Paul Ehrlich Institute (PEI) analyzed 122 such antigen tests for their suitability in November – even before the spread of the omicron variant [medrxiv.en.org]. 26 quick tests failed. Only four of the 122 tests also detected a moderate viral load, as is typical at the beginning and end of a Corona infection. Here [spektrum.en] you can check your quick test and see in an overview how well it performed in PEI’s test.

Questions and answers – What makes the Corona variant Omikron so special

New mutant, double worries. The omicron variant is likely to spread more rapidly and may in some cases defeat the immune defenses of vaccinated individuals. But the research does not only hold bad news ready. By Haluka Maier-Borst

How well do quick tests work with the new omicron variety?

The rapid tests also work with omicron – but not always as early as would be desirable. Shortly after the first omicron cases appeared in Europe, several virologists tested whether rapid antigen tests could also detect this variant. Your result: Yes, this variant is also recognized. The British health authorities soon confirmed this [service.gov.uk].

In the meantime, however, the Paul Ehrlich Institute (PEI) also points out that antigen tests were not developed for the reliable diagnosis of a Corona infection, but to identify people with a very high viral load quickly and easily. The PEI states that the tests can only detect an infection, even with the Omikron variant, if there is a high viral load at the time of testing.

But: In principle, according to PEI, the majority of the Corona rapid tests offered in Germany can detect the Omikron variant. The president of the institute, Klaus Cichutek, recently said on the ZDF "Morgenmagazin" television program, that the institute has now evaluated more than 250 test products to a higher level of sensitivity and that at least 80 percent managed to reach this level. The PEI provides the list of test results on its website as a pdf [pei.en].

Health Minister Karl Lauterbach (SPD) had a "positive list" Announced for quick tests that can detect omicron well. The data mentioned by the PEI are an interim status, a health ministry spokesman explained. The compilation of the complete list lasts. He reiterated that rapid tests do not provide 100 percent certainty, but they do provide more security in everyday life.

Interview [Corona infection – When vaccinated people should test themselves

Corona numbers are on the rise again. Vaccinated are well protected from severe courses, but they can still get infected. Immunologist Andreas Radbruch explains why a quick test can be useful in certain situations.

How reliable are PCR tests with the Omikron variant?

PCR tests are more accurate and therefore more reliable than antigen tests. This is due to the way they work. In PCR tests, the genetic material of the pathogen is detected in the laboratory in complex steps with the help of a special PCR device, explains Martin Roskos from the laboratory service provider Synlab Germany. Antigen tests detect proteins characteristic of the virus – through self-tests or rapid tests.

However, the difference in reliability is "quite substantial" for all antigen tests compared to the PCR test, especially for asymptomatic corona infections, Andreas Bobrowski, a laboratory physician from Lubeck, points out. "The main problem, which both the laboratory-based antigen test and the rapid test have, is simply that the sensitivity decreases significantly as the viral load decreases." This also applies to the Omikron virus variant.

Sensitivity is one of two values that play a role in the reliability of Corona tests. While specificity indicates how many uninfected people correctly receive a negative result, sensitivity indicates the proportion of those infected with the virus who actually correctly receive a positive test result. So what can be deceptive: A negative antigen test result does not necessarily rule out infection – and can also result from a low virus load at the time of testing.

What to do if the PCR test is not ready in time

Do quick tests work worse in vaccinated than in unvaccinated users??

This cannot be said. The Paul Ehrlich Institute currently assumes that unvaccinated and vaccinated individuals have a comparably high viral load at the peak of infection. At this point, rapid tests would be positive for both vaccinated and unvaccinated people.

However, vaccinated individuals excrete viral load over a significantly shorter period of time compared to unvaccinated individuals, according to the Paul Ehrlich Institute. This means that a Corona infection can be detected for a shorter period of time in vaccinated persons using a rapid test – but they are also contagious for a shorter period of time.

What should I keep in mind when doing a self-test at home?

You should store rapid tests according to the instructions on the packaging and keep them test at room temperature. If it is too cold or too hot, this can lead to false-positive or false-negative results. For most self-tests, you need to swab the front of your nasal mucosa with a cotton swab. However, your rapid test may work differently. Therefore, please read the package instructions carefully and follow all steps.

Pay attention to the result of the test not to be read too early: Mostly it takes about 15 minutes. Please refer to the package insert for the exact times. Also in case of negative result you should remain cautious. Keep in mind that rapid tests require a relatively high amount of virus for the tests to show a positive result. Scientists such as the virologist Christian Drosten assume that rapid tests in the three days before the onset of symptoms in a Corona infection do not respond reliably [ndr.en].

So you could be infected, although your quick test showed a negative result. Therefore, the Federal Ministry of Health continues to advise to keep your distance, wear a mask and ventilate indoors regularly [togethergegencorona.en]. How to ventilate most effectively is shown by the science format Quarks:

My quick test is positive, I can see two dashes. What should I do now?

Even if the second line is only faintly visible, the rapid test is positive. In this case, please stay at home and isolate yourself, because you are probably infected with the Corona virus. In rare cases, quick tests can also be false-positive.

Call your family doctor’s office or dial 116 117 to have the result of your rapid test clarified by a PCR test. You can get it for free on medical prescription.

a, only in Israel it has also been found that after the 3. Vaccination no significant immunization increase at the 4. Vaccination gives. After the 3. vaccination the goal is reached with this vaccine. There will be a new vaccine, and then we start again from the beginning. When the time is right, there will be a vaccine that is vaccinated 1 time in the fall/winter, as with flu protection.

That the test obligation for triple vaccinated and soon also the contact quarantine is omitted I find risky, since with a not small number of these Booster already up to a half year is ago. Why did not one at least coupled to a period of time (z.Ex. max. 3 months since booster)? Israel is not just for fun to the 4. Vaccination straight go over.

When I read this here so: It cannot be the sense of the thing to drive around crosswise because of a PCR test if a concrete suspicion exists. And whether ih one must pay it is also not obligatorily regulated.

My family doctor does not offer PCR test and does not let colds in the practice. Why . He works with junkies too.

To Berlin.de Berlin far PCR test sites are indicated. In Neukolln z.B. at the health office Blaschkoallee.

How is that with the account with capital PCR testers? Do I have to pay or is it enough to hand in the prescription??

About the quick test. At my employer 10 people tested positive in one day – all negative after PCR test.

With a positive test begins a madness. It is hard to find a general practitioner or hospital that will order a free PCR test. In Erkner it costs 80€. Otherwise you should drive to Beeskow, which is 100 km away.
So a reasonable detection and control of Corona can not work.

Exactly the same problem has a work colleague of mine
She has also tested herself several times with the self-tests from our work, each time positive
Although was the 2. STRIP not quite as clear, but it was there
She has dan own,or I think even 2 PCR tests made negative
Then she did 2 more of the self-tests and they were positive . Then she got another kind, the same, again positive
After that she did another PCR test, which was negative again . Now she says that unsettles the so that she no longer makes a self-test, is also understandable
However, I also do these self-tests,with me they are always negative, what is the reason for this, we can not explain
PS. She has tested herself several times in our presence

Since the 8.12. I test myself regularly at work. After the first positive result, I tested myself a second time. Also positive. So the next day a PCR test at the family doctor made, result negative. All other quick tests I did with two different manufacturers (last yesterday) were still positive. I do not know what to think about the tests. Even if the negative PCR result should have been wrong, after now 20 days the rapid tests should show nothing more. Anyway, I am at a loss at the moment and will go to the doctor again next week.

"everywhere, where rules according to 2G+ apply – for example in the culture and leisure area, at the hairdresser" – wrong. At the hairdresser applies 2G plus mask. A test that does not show correctly on vaccinated people is also worth much less than a well-fitting FFP2 mask. This simply has to be clear. As long as I don’t wear a mask, 2G is as safe as 3G or nixG, because I can be infected and I can infect others. Maybe I won’t get super sick, but infecting is possible. Only a FFP2 mask or staying away really protects!

Which tests are okay was published at Spektrum, the link to it is in this article from rbb you just commented on.

The text mentions that only 4 rapid tests have ever detected a moderate viral load, which can be.b. at the beginning of an infection, recognize. Can anyone tell me which rapid tests these are?? And then, unfortunately, the majority of rapid tests make little sense as routine and preventive testing! Disturbingly

If you have a red alert in the Corona app, you are entitled to a free PCR test. Should also be included in this article.
Among others at the official test centers in Berlin and also at private ones like z.b. coronatest.de

Yes, we have had this experience at work three times, that self-tests have given false alarm
On the other hand, people have also tested positive with self-tests, which were also positive
So I test myself regularly 2 to 3 times a week, never positive, and I personally would recommend it
Like" Steffen" already wrote
Better once too much than once too little
But everybody has to decide for himself

The test cannot distinguish between a covid infection and a vaccination.
The value then shows only the antibodies or. how well / poorly you are protected (most likely).
However, this test is also very controversial among doctors and scientists.

Yes, the rapid antigen tests have been known to look at more often and set off false alarms. The cause for this can be for example similar infections. Then only the PC test can really exclude a Corona infection. Nevertheless, one should continue to do the rapid tests to exclude a possible infectivity. Better warned once too much, than once too little.

Don’t be mad at me, but it reads like you’ve never done rapid testing before?

A "red" appears line if you have tested correctly and a second line if you are positive.
However, there is no "A for Alpha "D for Delta or "O for Omikron.

No, of course it cannot. For this, the genetic material of the virus must be examined in the laboratory, so-called sequencing. The tests themselves always show only components of the virus, not the exact type of virus.

Thanks for the summary.
But you know: Since I was positive 2x in a row with a self-test one morning at work, my colleagues and I were isolated as a result and quite a psycho and stress movie was triggered in my head (what does that mean now? When do the symptoms appear? How do I get my kids, my groceries, etc. organized?? etc.), but then the PCR test came back negative, I don’t pay much attention to these self-tests – and honestly don’t even do them anymore if I don’t have to.
This is my experience.

Currently, contact reduction is the best way to slow down the spread of the disease.
If you want to know if you are immune, the most sensible thing is to have an immunoglobulin test done by your family doctor. Blood is taken for this purpose.
This way it is possible to determine whether the vaccination has worked or not. Whether one has perhaps already had contact with COVID and is already immune or the BAU value determined in the laboratory is so high that one should wait with the next vaccination!
